IN CONSIDERATION of being allowed to participate in O.G. Skate Camp activities, the undersigned, on behalf of themselves, their child/ward, and their heirs, assigns, and personal representatives, agree to the following: *

1. Acknowledgment of Risks I acknowledge that skateboarding and related activities involve inherent risks, dangers, and hazards, including but not limited to: falls, collisions, equipment failure, terrain conditions, weather, and the acts or omissions of others. I understand that serious injury, disability, or death may result from participation in these activities. 2. Voluntary Participation I knowingly and voluntarily allow my child/ward to participate in O.G. Skate Camp activities with full understanding of the risks involved. 3. Assumption of Risk I expressly and voluntarily assume all risks of injury, harm, loss, or damage to my child/ward arising from participation, whether caused by negligence or otherwise. 4. Release and Waiver To the fullest extent permitted by Colorado law, I release, waive, discharge, and covenant not to sue O.G. Skate Camp, Othello Clark, staff members, contractors, volunteers, property owners, sponsors, and affiliates ("Releasees") from any and all claims, demands, liabilities, actions, causes of action, or suits arising out of or related to any injury, loss, or damage sustained by my child/ward during participation. 5. Indemnification I agree to indemnify and hold harmless Releasees from any loss, liability, damage, or cost incurred as a result of my child’s/ward’s participation, whether caused by negligence or otherwise. 6. Medical Consent I authorize O.G. Skate Camp staff to administer or secure emergency medical treatment for my child/ward in the event of injury or illness. I understand that I am responsible for any medical costs incurred. 7. Image Release I grant permission to O.G. Skate Camp to use photos, videos, or other media recordings of my child/ward for promotional purposes without compensation. 8. Severability If any provision of this agreement is found invalid, illegal, or unenforceable, the remaining provisions shall continue in full force and effect. 9. Governing Law This agreement shall be governed by and construed in accordance with the laws of the State of Colorado.
